1. 取得する情報 / Information collected

氏名、連絡先、希望言語、目標、運動経験、予約情報、練習環境、痛み・けが・治療に関する自己申告、フォーム動画、支払状況、お問い合わせ内容を取得する場合があります。オンライン決済を利用する場合、決済カード情報はStripeが取り扱い、Seiji Workoutは完全なカード番号を保管しません。

We may collect names, contact details, preferred language, goals, training history, booking data, practice-space details, self-reported pain or treatment information, form videos, payment status, and enquiries. When online payment is used, Stripe handles payment-card data; Seiji Workout does not store full card numbers.

2. 利用目的 / Purposes

We use information to assess fit and safety, manage bookings, deliver and improve coaching, process payments and refunds, meet legal obligations, and respond to enquiries. Progress footage or testimonials are published only with separate permission.

3. 痛み・健康情報と動画 / Health information and videos

痛み、けが、治療歴等は、安全な指導範囲の判断のために本人の同意を得て取得します。医療診断には使用しません。指導に必要な動画と、広告・実績として公開する動画の同意は分けて取得します。顔を公開する必要はありません。

Self-reported pain, injury, and treatment information is collected with consent to assess safe coaching scope, not to diagnose. Consent for coaching review is separate from consent to publish media. Clients do not need to show their face publicly.

4. 使用するサービス / Service providers

お問い合わせフォームはFormspree、無料体験セッションの予約はGoogle Calendar、ウェブサイトの配信はRenderが処理します。同意した場合のアクセス解析にはGoogle Analyticsを使用します。コーチングの運営には、本人の選択または同意に応じてGoogle Meet、Drive、LINE、WhatsApp等を使用する場合があります。オンライン決済を利用する場合、カード情報はStripeが処理します。各事業者はそれぞれの規約とプライバシーポリシーに従って情報を処理します。

Formspree processes contact-form submissions, Google Calendar processes free introductory-session bookings, and Render hosts the website. Google Analytics processes website usage data when analytics consent is granted. Depending on the client's choice or consent, coaching may also use Google Meet, Drive, LINE, WhatsApp, or similar tools. When online payment is used, Stripe processes card data. Each provider processes information under its own terms and privacy policy.

5. 第三者提供 / Sharing

法令上必要な場合、生命・身体の保護に必要な場合、または業務委託先へ必要な範囲で提供する場合を除き、本人の同意なく個人データを第三者へ販売・提供しません。

We do not sell or disclose personal data without consent except where legally required, necessary to protect life or safety, or necessary for service providers acting on our behalf.

6. 保管と安全管理 / Retention and security

情報は提供目的に必要な期間と法令上必要な期間に限り保管し、アクセス制限、アカウント保護、適切な共有設定等の合理的な安全管理を行います。不要になった情報は削除または匿名化します。

Information is kept only as long as needed for service delivery and legal obligations. We use reasonable account, access, and sharing controls, then delete or anonymize information when no longer required.

7. Analytics・Cookie / Analytics and cookies

ウェブサイト改善のため、同意した場合に限りGoogle Analyticsを使用します。分析用Cookieは初期状態では無効で、「Cookie設定」から許可した場合にのみ有効になります。選択はフッターの「Cookie設定」からいつでも変更できます。フォームに入力した内容はGoogle Analyticsへ送信しません。

We use Google Analytics to improve the website only when you consent. Analytics cookies are disabled by default and become active only after you accept through Cookie settings. You can change your choice at any time through Cookie settings in the footer. Form contents are not sent to Google Analytics.

8. 開示・訂正・削除・お問い合わせ / Your choices

本人の情報について、法令の範囲で開示、訂正、利用停止、削除を請求できます。ウェブサイトのお問い合わせフォームからご連絡ください。
