1. 取得する情報 / Information collected

氏名、連絡先、希望言語、目標、運動経験、予約情報、練習環境、痛み・けが・治療に関する自己申告、フォーム動画、支払状況、お問い合わせ内容を取得する場合があります。決済カード情報はStripeが取り扱い、Seiji Workoutは完全なカード番号を保管しません。

We may collect names, contact details, preferred language, goals, training history, booking data, practice-space details, self-reported pain or treatment information, form videos, payment status, and enquiries. Stripe handles payment-card data; Seiji Workout does not store full card numbers.

2. 利用目的 / Purposes

• 個別プログラム、フォームレビュー、進捗確認、連絡のため
• 決済、返金、会計、法令対応のため
• サービス品質、ウェブサイト、案内内容の改善のため
• 本人の別途同意がある場合に限り、実績・動画・感想を公開するため

We use information to assess fit and safety, manage bookings, deliver and improve coaching, process payments and refunds, meet legal obligations, and respond to enquiries. Progress footage or testimonials are published only with separate permission.

3. 痛み・健康情報と動画 / Health information and videos

痛み、けが、治療歴等は、安全な指導範囲の判断のために本人の同意を得て取得します。医療診断には使用しません。指導に必要な動画と、広告・実績として公開する動画の同意は分けて取得します。顔を公開する必要はありません。

Self-reported pain, injury, and treatment information is collected with consent to assess safe coaching scope, not to diagnose. Consent for coaching review is separate from consent to publish media. Clients do not need to show their face publicly.

4. 使用するサービス / Service providers

運営上、Formspree、Google Forms、Sheets、Calendar、Meet、Drive、Analytics、Stripe、LINE、WhatsApp、GitHub、Renderを使用する予定です。お問い合わせフォームの送信内容はFormspreeが処理します。各事業者はそれぞれの規約とプライバシーポリシーに従って情報を処理します。

Operations may use Formspree, Google Forms, Sheets, Calendar, Meet, Drive, Analytics, Stripe, LINE, WhatsApp, GitHub, and Render. Contact-form submissions are processed by Formspree. Each provider processes information under its own terms and privacy policy.

5. 第三者提供 / Sharing

法令上必要な場合、生命・身体の保護に必要な場合、または業務委託先へ必要な範囲で提供する場合を除き、本人の同意なく個人データを第三者へ販売・提供しません。

We do not sell or disclose personal data without consent except where legally required, necessary to protect life or safety, or necessary for service providers acting on our behalf.

6. 保管と安全管理 / Retention and security

情報は提供目的に必要な期間と法令上必要な期間に限り保管し、アクセス制限、アカウント保護、適切な共有設定等の合理的な安全管理を行います。不要になった情報は削除または匿名化します。

Information is kept only as long as needed for service delivery and legal obligations. We use reasonable account, access, and sharing controls, then delete or anonymize information when no longer required.

7. Analytics・Cookie / Analytics and cookies

ウェブサイト改善のためGoogle Analyticsを使用する予定です。分析用Cookieは、公開時に表示する選択画面で同意した場合にのみ有効化します。ブラウザ設定からCookieを削除することもできます。

We plan to use Google Analytics to improve the site. Analytics cookies will be activated only after consent through the choice shown at launch. You can also delete cookies through your browser.

8. 開示・訂正・削除・お問い合わせ / Your choices

本人の情報について、法令の範囲で開示、訂正、利用停止、削除を請求できます。ウェブサイトのお問い合わせフォームからご連絡ください。

You may request access, correction, restriction, or deletion where applicable. Use the contact form on the website.
